Life Impact Rehabilitation Services is a community-based organization that offers
Substance Abuse - IOP (Intensive Outpatient Services). We aim to provide the best
services to assist individuals and their families in accomplishing their goals. The
services will assist each of our clients to overcome obstacles hindering them from
creating healthy and nurturing environments that will allow them to become independent
and excel. These services will be available in the communities in an intensive outpatient
capacity while partnering with local agencies that may be able to provide the individual
receiving services with services that Life Impact Rehabilitation Services cannot offer.
Life Impact Rehabilitation Services focus is to provide cost-effective, quality service.
The clinicians (Licensed Substance Abuse Counselors, Qualified Mental Professional-A,
Qualified Mental Health Paraprofessional) and the licensed mental health professional
providing the service will meet the educational/experience regulations set by the
Department of Health Professions, the Department ofMedical Assistance Services and
the Department of Behavioral Health and Developmental Services.
1. Life Impact Rehabilitation Services Intensive Outpatient services (ASAM Level 2.1) will be a structured program of essential addiction education skilled treatment services for adults, children and adolescents delivering a minimum of 3 service hours per service day to achieve 9 to 19 hours of services per week for adults and 6 to 19 hours of services per week for children and adolescents. These services can also be provided on the weekend and those hours are part of the weekly hours as long as they do not exceed the maximum hours. This service is provided to participants who do not require the intensive level of care of inpatient, residential, or partial hospitalization services, but they do require more intensive services than outpatient services.
2. Life Impact Rehabilitation Services will also demonstrate the following service components in the participant’s ISP as medically necessary, through provision of services or through referral:
A. Psychiatric and other individualized treatment planning.
B. Individual, family and/or group psychotherapy.
C. Medication management and psycho-educational activities.
D. Requests for a psychiatric or a medical consultation shall be available within 24 hours of the requested consult by telephone and preferably within 72 hours of the requested consult in person or via telemedicine. Referrals to external resources are allowed in this setting.
E. Psychopharmacological consultation.
F. Addiction medication management provided on-site or through referral.
G. 24-hour emergency services available.
H. Occupational and recreational therapies, motivational interviewing, enhancement, and engagement strategies to inspire a participant’s motivation to change behaviors.
I. Medical, psychological, psychiatric, laboratory, and toxicology services, which are available through consultation or referral, as indicated in the member’s ISP. For participants who have not been screened for infectious diseases within previous 12 months, screening provided on-site or referral for screening of infectious diseases such as HIV, Hepatitis B and C, and tuberculosis at treatment initiation.
J. Withdrawal management services will be provided as necessary by qualified staff either on site or through referral. Life Impact Rehabilitation Services will refer to the ASAM Criteria text for Intoxication/Withdrawal Management guidelines.
3. Service Goals:
At a minimum, this level of care will provide a support system including
medical, psychological, psychiatric, laboratory and toxicology services within 24 hours by
telephone or within 72 hours in person. Emergency services are available at all times, and
the program should have direct affiliation with more or less intensive care levels and
supportive housing. Life Impact Rehabilitation Services intensive outpatient services are designed to help participants
achieve changes in alcohol and/or drug use and addictive behaviors and often address issues that have the potential to undermine the
participant’s ability to cope with life tasks without the addictive use of alcohol, other drugs, or both.
Description of Care and Treatment Services:
SUD intensive outpatient treatment provides intense treatment with the same supportive,
nurturing care and wellness professionals as an SUD inpatient program. SUD intensive
outpatient programs typically include daylong treatment, similar to a residential program, but
without the overnight stay.

Life Impact Rehabilitation Services will promote Substance Use Disorder (SUD) services by:
1. Providing structured activities and allow individuals to make individual decisions
2. Providing ongoing support and guidance
3. Providing techniques that will encourage independent living skill goals
4. Offering treatment team meetings and collaborative problem-solving with everyone, including family, that is a part of the individual’s substance use treatment plan. This will be conducted in a comfortable and safe environment. Sessions will focus on what the individual receiving services want, needs, and solutions.
5. Address deficits in skills that may be preventing an individual from accomplishing goals
6. Address any Activities of Daily Living (ADLs)
7. Address Relapse Prevention
8. Integration of family-based counseling interventions into SUD Treatment
Life Impact Rehabilitation Services believes and emphasizes that effective treatment can help participants addicted to drugs stop using, avoid relapse, and successfully recover their lives. An individual treatment and services plan must be assessed continually and modified as necessary to ensure that it meets his or her changing needs.
Characteristics and Needs of Individuals Receiving Services
As an Intensive Outpatient service provider, Life Impact Rehabilitation Services will meet the ASAM Level 2.1 service
components. The following service components will be assessed and monitored weekly and will
be provided in accordance with the ASAM Criteria, as directed by the participant’s ISP and
based on the participant’s treatment needs identified in the multidimensional assessment below.
There are many principles that guided the development of The ASAM Criteria. Some of these principles can better explain the mindset of a physician or care provider and help Life Impact Rehabilitation Services understand how these criteria are used in determining the best treatment services for each participant.
